Observation

A plastic bag is seen on a mattress in a crib. A cord was seen wrapped around a large teddy bear's neck in corner the infant pen/area. A second fridge/freezer is located outside in the play area with standing water surrounding it. It does not have a way to prevent access or entrapment. A toy box full of toys was seen outside. The toys were covered in mold and mildew.
