There were no medication authorizations on file for the two medications. 3)a)ii)A) There was one non-prescription medication in room 2X that was not labeled with the child's first and last name and was not accompanied with the medication authorization form with the expiration date, medical need, dosage amount, age, and length of time to give the medication.
