Observation

Window blinds not secured to sills were in use in toddler room. They were removed prior to my departure. Plastic bags are accessible to children in some rooms. There is a rusty, protruding bolt on the diaper changing table in the preschool room. Chairs are routinely stacked on top of tables during nap time to make space for mats and these could easily fall and topple on children. Flaking paint and rusty parts of play equipment are present in all licensed spaces and on the outdoor play equipment. Not all outlets are TR type or blocked with tamper-resistant covers. Outlet near sink in kitchen is not TR/GFCI type. Extension cords for fans were in place as a more permanent solution in several classrooms. Some fans, CD players, speakers, were plugged in at outlets near sinks.
