2c) Cords capapable of forming a loop accessible to children were found in the following rooms: SE Room had A/C cord hanging from being mounted on the wall NE Room cellphone cord on toy shelf SW Room cellphone cord on counter NW Room had A/C cord hanging from being mounted on the wall 2d) Plastic bags were found in the following rooms accessible to children: SE room had plastic bags on changing table, Toddler Room had plastic bags on changing table accessible to children NE Room had plastic garbage bags on diaper changing table SW Room had plastic garbage bags on bookshelf 3f) Cubbies were not secured to the wall in SE Room 3g) Licensors observed bikes on smaller playground outside that had rust on them, larger climber on older playground had a blue slide that had a screw/nail missing from the top securing it to the structure. 4c) Ceiling light cover in NE room was cracked and hanging down leaving bulbs exposed 5a) Outlets in all classrooms and hallways were not tamper resistant 5b) Outlets near sinks and accessible to children were not GFCI/TR
