(3)(b) Assistant Ruth Matias-Tenchipe has not completed Child Care Basics and is left unsupervised with child care children while provider off site transporting school children on 1/23/2023 and on other occasions as reported by Ruth Matias-Tenchipe and provider Trisha Shoop. (12) 12 year old household member observed serving breakfast to childcare children, has no current food handler card and is under minimum age requirement to work as an aide or volunteer.
