-0165 (2) There are indoor and outdoor cords which are not secured, close to the second exit door. -0165 (2) (d) Plastic bags located in the bathroom were not secured during onsite visit. -0165 (2) (f) There are no stickers on the glass of the sliding door; both sides need it. -0165 0165 (3) (a) In the outdoor area, are some metal "stakes," which have sharp and pointy edges. -0165 In the outdoor area was a hose on the ground, and it is a tripping hazard. There was also garden(3) (e) groundcover material that was not leveled and it is a tripping hazard too.
