110-300-0165(2)(a): TR caps were observed in the outlets in the baby classroom. 110-300-0165(2)(d): Plastic bags were accessible to children in the unlocked drawer beneath the diaper-changing station in the 'Bird' classroom. 110-300-0165(3)(c): The metal panels on the bathroom doors in the 'School Age' classroom were missing screws, making them partially detached. 110-300-0165(3)(g): Furniture in some classrooms and bathrooms exhibited flaking paint. The garbage container in "Bird" classroom had a crack on the lid. Additionally, the mats and rugs in every classroom were observed to be stained and visibly dirty. Two mats observed in "School age" classroom had torn edges. 110-300-0165(2)(c): Multiple cords were observed on the second shelf from the bottom of the metal shelf in the 'School Age' classroom. 110-300-0165(5)(a): The outlets in the classrooms were not equipped with TR covers.
