110-300-0165 (2)(c)- In Room 8, there was a cord long enough to form a loop accessible to a child. Provider corrected this at the time of the visit by securing the cord to the wall. 110-300-0165(2)(d)- There were plastic bags accessible to children in the bathroom. Provider corrected this at the time of the visit by moving the bags out of the reach of children.
