Observation

The provider does not have a file for her own son who occasionally counts in capacity. Nor did she have a file for the five year old family member visiting the home today. Dental care provider or parent/guardian's preferred facility for dental treatment is missing from three of six children's enrollment record. Date of last dental exam was missing from one child's enrollment record. Consent to medical care and treatment of minor children was not complete for three of six children enrolled.
