Observation

Fire extinguisher in the licensed space was not mounted, placed on the kitchen counter. Fire extinguisher will need to be mounted according to WAC 110-300-0170 Fireplace, heating device is located in the licensed space. Provider will need to submit a written statement that fireplace will not be used at any time.
