Observation

Licensor observed bed sheets were too small for the infant mats causing the mats to be0290 (3)(c)(d)(f) folded not allowing the mat to be a snug fitting mattress. Fitted sheet used did not fit properly on the infant mats restricting the mats to fit firmly on the equipment. Sheets did not appear to be laundered.
