110-300-0165(2)(c) At the time of the visit there was a cord from a device near the entrance door that was accessible and could form a loop. There was a hose in the licensed outdoor space that was in the floor and could form a loop. There was a cord on the kitchen counter that was accessible. 110-300-0165(2)(d) There was a plastic bag in one of the personal cubbies near the entrance. 110-300-0165(3)(a) at the time of the visit there was multiple sharp nails on the floor in the licensed outdoor space and on a wooden panel that was on the floor.
