0165(2)(d) - Licensor observed a crib mattress to still have a plastic bag on it with a sheet covering the bag. 0165(3)(a) - Licensor observed multiple pallets against the fence in the backyard with protruding nails 0165(3)(f) - Licensor observed two shelves in the entry way of licensed space to not be anchored to the wall.
