During onsite inspection, Licensor reviewed MERIT on 03/19/2024 and onsite staff records and observed that Lead Teacher DA’s Bloodborne Pathogens Training expired on 03/05/2022 and has not been renewed. (10) During onsite inspection, Licensor reviewed MERIT on 03/19/2024 and onsite staff records and observed that Provider FD’s (01/11/2022) and DA’s (03/16/2023) Food Handler Permits are expired and have not been renewed. (12) During onsite inspection, Licensor reviewed MERIT on 03/19/2024 and onsite staff records and observed that Provider FD’s (10/03/2023) and DA’s (03/14/2022) Safe Sleep trainings are expired and have not been renewed.
