110-300-0165(1) Licensee had several building materials in the outdoor play yard area accessible to the children including, boards, nails, a large seven-foot hole in the back yard alongside the house that a child could fall and get hurt or trapped, brooms, dawn dish soap, large scissors, and shovels. 110-300-0165(2)(c) Licensee had phone cords and a computer wires accessible to the children. 110-300-0165(2)(d) Licensee had several plastic bags accessible to the children in the play room under the changing table and one in the bathroom under the cabinet. 110-300-0165(5)(a) Licensee had three outlets in the play room and one in the hallway without automatic shutters, tamper resistant or a cover accessible to the children.
