During the onsite montor visit this licensor observed an Epi-pen that did not have the presciption label with the child's first and last name, the date the prescription was filled, the name and contact information of the prescribing health professional, expiration date, dosage amount, and lengh of time to give medication, and instructions of the administration and storage.
