0165 (2) (a) It was observed that child put a small rounded toy in his mouth. Correction: Licensor informed provider that child put the toy in his mouth, provider asked the child to spit out the toy. Provider removed the toy. 0165 (3)(d) It was observed that fridge is accessible to chidden 0165 (3)(f) - Fire extinguisher is not mounted/ secured to the wall 0165 (4)(c) Exposed lights observed in the play room and infant room. Correction: Provider removed the lights during the visit
