Observation

Diaper changing pad had safety straps and was located on top of a laundry appliance and did not have a protective barrier at least 3.5 inches on all sides from where the child would lay. The diaper changing area did not have a designated diaper disposal container that was hands-free, lined, or within arm's length of diaper changing area.
