Observation

0241(1)(a) - At the time of inspection, the eating table had not been cleaned and sanitized after the last meal. Grapes, toys, and raised stains were observed on the eating table. 0241(1)(g) - During the inspection, this licensor observed grime and stains on multiple toys located in the Main Room. 0241(10) - During the inspection, this licensor observed stains, crumbs, trash, and other debris on the floor of the Main Room, hallways, restroom, and the Sleep Room. 0241(11) - During the inspection, this licensor observed multiple stains on the large rugs in the Main Room. 0241(3) - During the inspection, this licensor observed multiple urine stains on the mattress used in the damaged Pack 'n Play. 0241(6) - During the inspection, this licensor observed the following in the restroom: The handwashing sink could barely drain water. This led to a large accumulation of dried gunk around the drain. Dirt, stains, and other debris on the floor. Crayon marks, dirt, and stains on the restroom door and walls. Large occupied spiderwebs in the ceiling corners. Cockroach carcasses were observed spilling out of the light fixtures. No 3-step cleaning bottles. No handwashing soap was available. There was soap underneath the sink, but it had not had it's seal broken for use. Damaged floor trim with chipping paint. Large cracks on the walls. 0241(7) - During the inspection, this licensor observed the diaper changing pad to be covered in multiple yellow stains.
