2) Provider did not inspect for fire safety using state fire marshal form 3b) Provider's fireplace was covered with wood boards and there were toys and a playpen within 3 ft of fireplace 3f) Provider will not use fireplace and did not have written statement that fireplace will not be used at any time 3g) Room down hallway used for sleeping did not have smoke detector 3i) Top of fire extinguisher was mounted 68" off ground (more than 5 ft off ground)
