During onsite inspection, Licensor observed Provider IA unlock the emergency exit door (sliding glass door) to access the licensed outdoor play area. (3)(a) During onsite inspection, Licensor observed the emergency exit sliding glass door blocked with a shelf, toy shopping cart, toy wagon, and toy baby stroller, and a small table and chair (also observed to be locked from inside) and observed Provider IA move the items to access the licensed outdoor play area.
