2)d) There is a plastic bag in an open bag hanging in the hallway that is accessible to children in care. 3)g) There were two trikes in the outdoor play area that were not in good condition. One was missing the handlebar grips, posing a hazard of injury if a child tipped over while riding, and the other one had a missing pedal. 4)g) The back porch, where children exit for outdoor play, is 20" from the paved walkway below and there is no guardrail in the section without steps. 5)a) The cover on the outlet on the back of the house was not secured today. The outlet is not tamper-resistant and was accessible to children.
