Observation

MERIT was not up to date there was one staff member, H.A who stopped working at the program 2 weeks ago acocrdng to the provider but was still attached to the facility page. Another staff member named S.H whom had not worked at the site for 6 months according to the provider and still was attached to the facility page. The provider did not have files for KBG and I.G whom live on the property. H.H's file did not have application/resume and job title.
