During onsite inspection, Licensor observed an individual onsite not listed on the MERIT Facility Site page, Provider NA stated person’s name was FM (later during inspection NA stated name the person’s name was FA), and person would not identify themselves to Licensor (and unable to see if they met the background check requirement).
