0145(1) two egress area are accessible and pose a fall hazard, grass overgrown does not allow for outdoor play 0145(4) insufficient outdoor equipment for pre-school and school age children 0145(8) fence in outdoor area has large gap/dig hole at bottom of fence and gate has large gap at bottom right side both larger than 3.5 inches
