110-300-0280(1) Licensee was missing date on one prepared bottle in the infant room #1s refrigerator. 110-300-0280(2)(b) Licensee had a bottle being prepped three feet away from the diaper changing location while licensors were onsite in infant room #1. 110-300-0280(2)(c) Licensee did not have a barrier to prevent cross contamination from the diaper changing area to a bottle prepping location in infant room #1 while licensors were on site.
