Observation

Licensor observed outlet plugs smaller than three-quarter inches in outlet plugs. Plugs are already tamper-resistant, and not needed as they are a chocking hazard. Provider removed some of the plugs during time of visit. Licensor observed non-latex gloves inside of bathroom cabinet that were at children's reach and had no lock. Provider removed it immediately during time of visit. Licensor observed a door in licensed bedroom that did not have screws on the top part and was falling over.
