Children files were missing the sign off for the handbooks. Current staff, provider's daughters are missing documentation that the Disaster Plan has been reviewed. Disaster Drill log area read the months February, April, July and November but did not have an actual day written in. In the area that read February it also read 5 minutes but did not identify the drill conducted.
