Observation

-0165 (2) (a) In the infant room there was small debris on the floor, and if ingested, those are choking hazards. 0165 (2) (c) There were cords that were not secured, in all of the classrooms. -0165 (2)(d) There were bags in the bathroom classroom #3, counter that were not secured during child care hours. The bags on cabinet above, were not secured either with the safety device. 0165 (3) (c) There was broken/cracked equipment in the outdoor area. The playhouse is cracked below roof section. Tricycle had broken seat and two other riding toys, had cracked wheels. -0165 (3) (f) Toys storage shelves in all classrooms inspected, were not secured. -0165 (3) (g) There is chipping paint in the bathroom wall located in room #3. There is peeling paint also in the toy shelves in classroom #2 as well as the school age classroom; by the side door. -0165 (4) (c) There is no cover on ligh fixture located in classroom #4, and in the bathroom in classroom #3.
