2) Provider did not inspect for fire safety using state fire marshal form 3b) Provider's fireplace was blocked with cardboard within 3 ft from fireplace 3f) Provider did not submit to department a written statement saying they would not use the fireplace at any time. 3i) Fire extinguisher handles were mounted higher than 5' off the ground and there is no proof of purchase or when fire extinguisher was last serviced.
