(3) Licensor was not able to observe completed childcare basics for a staff member employed more than three months. (8) Licensor observed some staff had expired safe sleep training. (11) Licensor was not able to observe the current CPR/first aid certification for one staff member onsite, in ratio. (12) Licensor was not able to observe the current food handler's permit for a staff member.
