The staff information for this site is not accurate. None of the providers working on site today are affiliated with this site in MERIT. Multiple documents were found in on-site files that have not been entered in MERIT. Education has not been submitted to MERIT by Jay R. MERIT shows completion of SAB for Shirley O. but her certificate is not in her personel file. Richard R. was missing nearly all required items in MERIT and in his on-site file.
