Observation

There is an individual care plan on file for a child with an apparent food allergy, which is listed as a food allergy on both the health history on the child registration and on the individual care plan. 1) There are no written instructions from the child's health care provider and the individual care plan is not signed by the child's health care provider as required for food allergies. 2) There are two medications written in on the individual care plan to dispense in case of food allergy. The licensee stated she has one of the medications, but not the other medication referred to in the individual care plan.
