Observation

0165(2)(d) Plastic bags accessible to children in the women's bathroom across sanctuary and in the girl's bathroom diaper changing area. 0165(3)(d) Cabinets under the sinks in both men and women bathrooms across sanctuary posed entrapment hazard. Also found unsafe items accessible to children. 0165(3)(f) White shelf in the boys bathroom, heavy and unsafe items stored on the classroom shelves posed hazard if falling on children. 0165(3)(g) Observed broken items (containers, etc.) and dirty towels on the playground accessible to children. Hands-free diaper disposal container does not work properly. Observed the lid stayed open with soiled diapers in it. One of the classroom shelves support bracket was bent and shelf was tilted. 0165(5)(a) All outlets in the little lunch room and one outlet in the sanctuary appeared to be not tamper resistant.
