110-300-0165(2)(c) Licensee had a hose accessible in the play yard that could create a loop around a child neck. A cord to the air conditioner in the Pre-K room that could create a loop around a child neck, and on the back of the center door in the play yard there is a cord that is not attached to the wall anymore that could create a loop around a child’s neck. Licensee had cords to a noise maker and a speaker in the infant room and an air conditioner that could create a loop around a child’s neck, 110-300-0165(2)(d) Licensee had a plastic Walmart garbage back outside in the outdoor play area with garbage accessible. In the toddler room under the sink accessible, diaper bags in the changing area in the toddler room, plastic garbage bags in the Pre-K boy’s bathroom under the sink, and plastic bags accessible in the infant room two under the sink. 110-300-0165(3)(a) Licensee had the fencing back by the play water spicket are with wired sharp edges of fencing protruding and accessible to the children where they had been digging, there were also two large screws on the walking bridge that are protruding from the walking bridge. 110-300-0165(3)(d) Licensee had a storage room door open and accessible, making an open washer and dryer accessible to the children when walking through the hallway. 110-300-0165(3(f) Licensee had a large shelf in the toddler room by the toilet area that needs to anchored to the wall, a mini refrigerator in the infant room one needs to be secured to the wall, it is a tipping hazard. 110-300-0165(3)(g) License had flaking paint in the toddler room wall trim by the circle time area, chipping paint by the handwashing sink wall in the toddler room, the large covered wood area in the outside play area and walking bridge both having flaking wood and need to be retreated and sealed, there is flaking paint on the outside of the centers wall, and window area by the infant room one door area, there is paint chipping in the kitchen counter area one a shelve closest to the hallway, there is dry wall exposure on the wall as you come in the back door leading to the hallway right before you get to the infant room one area on the left hand side of the wall.
