Observation

(2) Provider was over capacity and over ratio. (3)(b)(c)(d) Staff were not aware of what children were doing at all times. Staff did not postition selves to supervise all areas. Staff did not visually check on children with little time between. (5)(b)- Door alarms were not activated during inspection. (5)(c) The children were not actively supervised during outside time. Children went outdoors alone. Staff supervising outdoors was observed on her phone and not closely supervising.
