The provider and the assistant is missing annual BBP training. The provider does not have documentation in MERIT for Basic/Initial STARS. Provider's trainings in MERIT for Mandatory Reporting Families Experiencing Homelessness, Abusive Head Trauma, Medication Management, and Emergency Prepardness all all "Self-Entered" and not "Verified".
