2)f)i) Child files did not have clear permission for off-site field trips and licensee reported taking the child on walks to the park. 4)e) Two child records did not have documentation of dental care provider name and contact information, and one of those records also did not have the medical provider name and contact info. If there is no medical or dental provider, it is acceptable to have the parent identify a facility they would prefer for treatment on the registration forms. 4)f) Child records did not include dates of children's last physical and dental exams.
