1)a) The magnetic safety latch was disengaged on the cabinet under the sink in the kitchen. There are toxic products stored there. 5) There was some maintenance/janitorial equipment in the outdoor play area where it was accessible to children, including a metal rake with hard metal points and another large rake for raking leaves.
