110-300-0165(2)(c) Licensee had cords in the Pre-K room that could create a loop around a child neck by the kitchen area, cords to a speaker and a radio were accessible in the toddler room, cords to a speaker in the Preschool room were accessible, a rope was accessible on the patio area that could create a loop around a child neck, an extension cord was accessible in a storage shed on the patio. and a hose in the infant play yard, that could create a loop around a child’s neck. 110-300-0165(2)(d) Licensee had plastic bags accessible In both bathrooms for children’s diapers, and in bathroom room there was a large plastic paper towel bag accessible, a large plastic bag was located in the patio area. 110-300-0165(3)(a) Licensee had a flag stake with a pointed edge that could cause a puncture next to the back door in the play area, railroad ties that were splintering edges in the playground area, an ac unit accessible that kids could stick their fingers in the fan and get cut, a metal stake with a pointed edge in the infant play yard, metal poles in infant area, cement blocks with sharp edges in the infant yard, a wood board that is splintering in the infant area, metal legs on a water table in the play are rusted and appear to be splintering metal. 110-300-0165(3)(d) licensee has two large windows well that do not have covers over them in the preschool play yard that are accessible to the children, and fall as an entrapment risk. 110-300-0165(3)(f) Licensee had two large bubble cars sitting on top of two large plastic blocks outside in the waddler play yard that pose a risk of falling on to a child. 110-300-0165(3)(g) Licensee had flaking paint in the pre-k room counter by the kitchen area, and on the play house in the pre-k room, two broken infant swings in the outdoor infant play yard,
