Observation

During a site visit for a complaint licensor became aware that staff member LD was in charge as the program supervisor at the time the allegation was alleged to have happened. On 1/23/23, licensor viewed LD on the MERIT facility site and it showed that it was confirmed (by the director) according to MERIT as LD starting in the program supervisor position on December 16, 2022. The department was not notified of staff member LD starting in the program supervisor position. This compliance issue was not part of the complaint but was discovered during the complaint process.
