Observation

110-300-0165(2)(d) - rolls of plastic garbage bags were observed located in the bathrooms on a level accessible to children. 110-300-0165(3)(c) - plastic toy trucks and carts with holes and brocken sharp edges were observed outside on the playground. 110-300-0165(3)(d) - fridges located in the classrooms were lacking safety locks. A saltwater aquarium located in the hallway, had a bottom drawer lacking a safety lock to prevent children from accessing the aquarium's filtration system. 110-300-0165(3)(g), the metal door frame in the bathroom in room #3 has exposed rust at children's level. 110-300-0165(5)(b) - the outlets observed near the sinks were not equipped with the GFCI outlet type.
