(3) 4 staff files that were reviewed were missing emergency contacts. Two staff files that were reviewed were missing MMRs. One staff file that was reviewed was missing a TB test result and had a document stating that this employee was supposed to go back to the clinic to have their result read and their food handlers card expired on 7/25/24. One staff file that was reviewed had a BBP training that expired on 2/11/24.
