Medication in grab and go bag missing medical authorization form. Medication in grab and go bag medical authorization form missing parent signature. Non Prescription medication must be in original package, and have a medication authorization form completed. Other non presciption such as teething gel must be accompanied with a written authorization from child's health provider.
