Licensor Bakowski observed that infant, M. Lakhotia, had diaper cream and did not have a medication authorization form completed. Licensor Bakowski observed that infant, S. Romley, did not have their full name labeled on the diaper cream and the name of the diaper cream on the medication authorization form. Licensor Bakowski observed that child, L. Shin, had an expired EpiPen and expired Zyrtec medication.
