2) State fire marshal inspection form was filled out on the front and not the back, signed, and dated. 3f) Provider has fireplace and did not submit to the department a written statement that fireplace would not be used at any time. 3i) Mounted fire extinguisher was 1A-10-BC and not a minimum of 2a-10-bc. Fire extinguisher did not have a receipt of purchase or tag showing when it was serviced last.
