WAC 110-300-0215(3)(a)(i) During the onsite monitor visit this licensor observed one epi-pen was missing the medical prescription label from the prescribed health care professional that includes the child's name, date the prescription was filled, name and contact information of the prescribing health professional, expiration date, dosage amount, length of time to give the medication, and instructions for administraion and storage.
