A child’s chair observed cracked and broken, diaper changing pad is missing some of the vinyl covering and is dark stained, doorknob loose on bathroom door and is a pinching hazard, 2nd doorknob is missing to a door in upstairs hallway, 2nd doorknob interior metal hardware is sharp and accessible to children posing a safety hazard.
