2) Applicant did not fill out annual fire inspection form 3a) Propane tank for grill was accessible to children outside (stairway was not gated off) 3b) Wall heaters were hot to touch and risk of burning children 3i) Fire extinguisher was on counter at time of visit and not mounted and fire extinguisher was too small (1A-10-BC)
