110-300-0165(2)(c) Licensee had cords to a fan upstairs in the living room, downstairs, vacuum cord accessible by the bathroom area. 110-300-0165(2)(d) licensee has plastic bags accessible under the sink downstairs this was completed while DF was onsite. 110-300-0165(3)(d) Licensee has no locks on washer and dryer in licensed space making these entrapment risk. 110-300-0165(3)(g) Licensee has chipping wood in the stairs leading downstairs, on both exit doors downstairs and leading to the back yard area.
